Cantwell joined Norwich City Academy at the under-10 level. He was then shortlisted for the Premier League Two player of the season award for the 2016-17 season. Cantwell was named as a substitute for two Norwich City games but did not make an appearance He then made his senior debut in January 2018 in an FA Cup match over Chelsea at Stanford Bridge. He later joined AFC Bournemouth on loan on January 31, 2022, to the conclusion of the season. Bournemouth then opted not to buy him at the end of his loan deal, and so he went back to Norwich. Cantwell signed a one-year deal extension with Norwich for the 2022-23 season.
Todd represented the England under-17 team at the 2014 Nordic tournament, making his debut over Iceland. He then scored in a draw over Finland. Cantwell was later included in the England under-21 team for the first time on August 30, 2019. He made his debut as a 61st-minute sub for Morgan Gibbs-White in the 2021 U21 EURO 2-0 qualifying victory against Kosovo at Hull City on September 9, 2019.
