Jake played his high school hockey at Eau Claire Memorial, prior to going to play college hockey at Wisconsin. In his first year with the Badgers, he suffered a severed tendon in his finger, cutting his season to just 26 games. Buffalo Sabres
The Sabres signed him to a 3-year deal on April 2, 2014, with the intent of having him join the Sabres’ active roster. McCabe then made his NHL debut the next day, skating 16:20 with the Sabres in a 2-1 defeat by the St. Louis Blues. On October 15, 2015, he registered his first career NHL goal in a defeat by the Florida Panthers. Following playing in almost all 82 games with the Sabres during the 2015-16 season, he signed a 3-year deal extension with the Sabres on June 30, 2016.
Jake then delivered a hit on Patrik Laine of the Winnipeg Jets on January 7, 2017. Laine suffered a concussion and exited the match after the hit but Jake was not disciplined or fined. He then concluded the regular season with career-highs in assists and points. During the 2017-18 season, he suffered a thumb and shoulder injury that cut his season short. Joining the 2019-20 season under new head coach Ralph Krueger, McCabe was named an alternate captain of the Sabres.

Jake left the Sabres as a free agent and signed a 4-year, $16 million contract with the Chicago Blackhawks on July 28, 2021.
